Record Locations

Special Education and Student Records are stored in one of four locations:

  • MIStar
  • Laserfiche
  • 80CA Folder
  • Ancillary Service Departments at Frederick Douglass Academy

To Review Special Education Records

Records may be requested by mail, or viewed by appointment. All requests for records require valid proof of identification.

Request Copies of Records by Mail

Copies of records may be requested by:

  • Student 18 or older
  • Parent or Legal Guardian
  • Other School Districts
  • Officer of a local, state, or federal judicial system
  • Government Agencies
  • Parent representatives

Please complete the Record Request Form and return it via one of the methods detailed below.

If you fax, email, or mail your request, please provide a clear copy of your identification.

If you are not the parent, you must also provide a signed release by the client, and evidence of representation.

Records may be accessed by appointment only. To set up an appointment, please complete the Access Records Form and return it via one of the methods detailed below.

To Submit Completed Special Education Record Forms

You can submit completed Special Education Request Forms through one of the following:

  • At your local school (current students only)
  • In person at:

    Hours: Monday through Friday, 8:30am — 4:00pm. Closed Thursdays.

Processing Time

Processing time is up to 15 business days once requests have been received by the Office of Special Education Placement and Compliance.

Please be advised that requests may require additional time to research, as these records are stored at an offsite location. Each case is unique; based on the age and quality of the student record, or other extenuating circumstances, special attention and time may be required to retrieve these documents. Your patience is requested as we work to retrieve these records for you.

Questions and Status Requests

Please contact the Detroit Public Schools Service Desk at (313) 576-0090, Monday — Friday, from 7am — 5pm. Please have the following information readily available when you call:

  • Student's name(s) while attending Detroit Public Schools
  • Student's date of birth
  • Name of all schools student attended (Elementary, Middle, High School, Night School, Second Chance, etc.)
  • Student's graduation date (month and year)
  • Student's home address while in school
  • Receipt number from the money order or cashier's check sent with the request
  • Date request(s) and payment were sent to the office
  • Address where the request(s) and payment were mailed
  • Current name and phone number
  • Address or fax number where you would like the requested item(s) returned

Acceptable forms of Identification

A clear copy of your current identification (I.D.) is required in order to obtain a copy of your student records.

The following documents are accepted as I.D.:

  • Insurance Documents
  • Mail with Identifying Name and Address

If you are under 18 years of age, a parent or guardian must request your records.

If you are 18 or older, you must request the record(s) or provide a written and signed documentation and a clear copy of your identification authorizing someone else to obtain a copy of your record(s). In addition, the person authorized to request a copy of your record(s) must also have a clear copy of their own photo identification as well.
