Student Records and Transcripts

We are located at 2001 West Warren Avenue, Detroit, Michigan, 48202 in the South Wing of the Frederick Douglass Academy in the Student Services & Data Management Complex.

Walk-ins are accepted from 8:30 am until 2:15 pm on Monday, Tuesday, Wednesday, and Friday. The walk-in counter is closed every Thursday.

Parents Know Your Rights

Transcript Request Information

You must provide a copy of your identification in order to obtain a copy of your transcript records. The following documents are accepted as I.D.:

  • Driver's License
  • State I.D.
  • Passports
  • Social Security Cards
  • Birth Certificates
  • Court Documents
  • Utility Bills, Medical Records
  • Mail with Identifying Name and Address

If you are under 18 years of age, a parent or guardian must request your records.

If you are 18 or older, “you” must request the transcript or provide a written and signed documentation and a copy of your identification authorizing someone else to obtain a copy of your transcript.

How to Request a Transcript

In Person (Student Services & Data Management Complex – South Wing only)

You may request a transcript record by coming into our office during our normal business hours.

  • To process your transcript request::
  • Take a transcript release form
  • Fill out the form and sign it
  • Submit the signed form at the counter when you are called for service

In Writing (Email, Fax, Drop-Box, US Mail)

Pick-up or download the transcript request form.

Please be advised that transcripts for closed schools may require additional time to research as these records are stored at the District’s archive repository.  Each case is unique and based on the age and quality of the student record; special attention and time are required to retrieve these documents.  Your patience is requested as we work to retrieve these records for you.

Education Records for a Student with a Disability or a Student Evaluated for Special Education Eligibility

Special Education and Student Records are stored in one of four locations:

  3. 80’s Folder
  4. Ancillary Service Departments at Fredrick Douglass Academy

There are two methods to review Special Education Records: Records may be accessed for viewing by appointment, or copies of the records may be requested ($5.00 fee).

Request for records can be requested by:

  • Student 18 or older
  • Parent or Legal Guardian
  • Other School Districts
  • Officer of a local, state or federal judicial system
  • Government Agencies
  • Parent representatives

Access Records Form

Records may be accessed by appointment only by completing the Access Records Form

All requests for records require proof of identification.

Photo ID. – i.e. driver’s license, state ID., passport. If the request is faxed, emailed or mailed, please provide a legible copy of your identification. If you are not the parent, you must provide proper written authorization.

Code of Conduct Record Request

If you require a Code of Conduct record, please contact the Office of Student Assistance and Intervention at (313) 873-4857 or (313) 873-4858 or fax to (313) 873-7971 attention Dr. Foster.

Adult Educaton or Practical Nursing Program Record Request

If you require an Adult Education or Practical Nursing Program record, please call the Office of Adult Education at (313) 852-1120 or (313) 852-1085.

G.E.D. Record Request

If you require a G.E.D. record, please call the Office of G.E.D. at (313) 596-7615 or (313) 596-0872.

State of Michigan CA-7 Work Permit

This is a work permit/age certificate related to the Youth Employment Standards Act 90 of 1978
